2006 AMIA Spring Congress
Registration
Payment Payment must be in US dollars and payable through a US bank. Accepted forms of payment include personal or company check, money order, VISA, MasterCard, or American Express. Registration will not be extended to any individual with an outstanding balance due to AMIA. Purchase orders from paying institutions will be accepted in lieu of the above, but attendees are responsible for payment of fees in the event the institution does not pay within 60 days after invoicing by AMIA. Purchase orders will not be accepted from institutions with outstanding balances. Cancellation Policy Written Cancellation of a congress registration does not constitute cancellation of a hotel room. Please be sure to handle both separately. Cancellation and refund requests received. By March 27 - Full refund minus $50 processing fee. Between March 27, 2006 and May 1, 2006 50% refund. After May 1, 2006 no refund will be issued. However, registrations can be transferred to another member of your organization. Transfer of Paid Registration Policy Registrants who are unable to attend the meeting may transfer their registration to another person. Transfer of registration must be in writing and received by the AMIA office by May 8, 2006 or presented on-site. If a member registration is transferred to a non-member, that person will have to pay the difference between the member and non-member rates.
