2010- • PatientSite / Co-InvestigatorI am investigating the usage patterns of our online patient portal and clinical messaging with respect to clinical demographic factors. This is a retrospective cohort study of 50,000 users, using Poisson regression.
2011- • Social Communication Patterns after Discharge / InvestigatorVia qualitative research methods, we are understanding how medical teams and patients/families can best communicate following hospital discharge. This project uses semi-structured interviews after discharge to identify communication needs and understand the role they play in post-hospitalization care-seeking.
• Digital Professionalism Development in Undergraduate Medical TrainingUsing a prospective cohort design, we are surveying third year students to determine if their perceptions of digital professionalism change as they grow into their role as doctor and professional. I designed cases and co-designed the survey instrument.
• An Experiential Learning Curriculum for Patient - Doctor Electronic CommunicationI created a curriculum based around a one hour-lecture and a one-hour small group tutorial to teach and model best practices for electronic communication. I am analyzing pre-post surveys, with the anticipated direction to create an online evaluation tool for preceptors. 2012- • OpenNotes Project – Opening Doctors’ Notes to PatientsI am co-creating materials with the OpenNotes group for physician education, and co-designing a research study that includes residents and fellows.

Narrative Report (limit to 500 words) My scholarly and innovative work is in the area of ambulatory systems, information management, team-based communication, and patient-provider communication, with a special focus on social media and social networking. My work aims to understand how communities of physicians and patients can engage with one another using electronic tools to maintain contact for both prevention and chronic disease management. Ultimately, I hope to see the tools and innovations embedded as part of health care redesign if shown to be beneficial. Given that the area of electronic doctor-patient communication is new, recent work in the forms of studies and editorials have focused on the concept of ‘digital professionalism,’ or how the medical profession can integrate newer technologies into their lives while maintaining trust (security, privacy) in the medical system. My work also looks at the role of healthcare disparities and technologies. With Bruce Landon (HMS Department of Health Care Policy) and collaborators in the Division of Medical Informatics, I am studying the 10-year experience of patient portal use and clinical messaging, in terms of understanding the users of non-users of this technology, and to what extent a digital divide exists that can lead to disparities in care. I have also developed and implemented a curriculum for medical residents for managing and responding to patients via clinical messaging. With Tom Delbanco and Jan Walker, I am collaborating on the OpenNotes project, which shares clinic visit notes with patients electronically. I am assessing how this impacts graduate medical education and clinical training. We are also using qualitative methods to understand communication patterns among patients and providers after hospital discharge, trying to understand where communication tools may be helpful in preventing unnecessary emergency visits and readmissions. We are also preparing an updated version of PatientSite, our patient web portal, with the goal of incorporating tools for patient-doctor collaboration and decision-making tools for patients. My teaching currently is focused on how physicians can incorporate new web tools in their practice in meaningful and safe ways, and how clinicians can collaborate with patients via patient portals and secure messaging. I have collaborated on a national course for clinician educators on Web 2.0, and also have taught for interns and residents on information management and electronic communication.
